Family Stabilization Coach/Supervisor

Summary: The Coach/Supervisor will oversee the day to day operations of the Family Stabilization Project including developing, planning and enhancing services as well as supporting the coordinators & peer support specialists.

 ESSENTIAL JOB FUNCTION:    Key Responsibilities

  • Serve as the family stabilization subject matter expert to lead, coach and educate the staff on every phase of the wraparound process and provide support and feedback to ensure that staff are implementing each phase with high-fidelity.

  • Ensure community and financial resources are used efficiently and explore alternative funding streams as appropriate

  • Co-facilitate Family Team Conferences and assist with complex cases when indicated.  Build relationships with family members, providers and the community.

  • Conduct utilization reviews of case records, ensure use of Wraparound fidelity tools and assessments are followed per the Wraparound model.

  • Actively participate in the recruitment, hiring and training of Wraparound staff;

  • Plan performance expectations, monitor and appraise the employee’s work in order to develop, counsel or discipline staff members.

  • Serve as a member of project workgroups as assigned.

  • Perform other incidental or related duties as assigned.
